Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider to care for patients within the scope of training and approved agreement, collaborating with other providers to ensure efficient patient flow and high satisfaction.

The APP will perform comprehensive histories, order labs and imaging as appropriate, diagnose and manage care, and provide health guidance to patients and families in a walk-in urgent care setting.

Responsibilities

• Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for patient condition.
• Formulate medical and nursing diagnoses and institute therapy or referrals as needed.
• Institute emergency measures and emergency treatment or stabilization in critical situations.
• Interpret and analyze patient data to determine status, management and treatment.
• Provide instructions and guidance regarding health care and health promotion to patients/family.
• Other duties and responsibilities as assigned.
