Job summary

American Family Care, Inc. is seeking an Advanced Practice Provider (APP) to care for patients within the scope of training and the approved agreement, as outlined by the governing board. The APP will work collaboratively with other providers and staff to ensure efficient patient flow and high satisfaction.

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• Board-certified by ANCC, AANP, or NCCPA; ER/UC experience preferred.
• Active PA or NP licensure in state of practice and state CS certificate as required.
• Able to multitask and work independently.
• DOT certification preferred.

Responsibilities

• Perform complete, detailed health histories and review patient records; develop medical assessments; order labs, radiology and diagnostics as appropriate.
• Formulate medical and nursing diagnoses and institute therapy or referrals to appropriate facilities and resources.
• Institute emergency measures and stabilization in cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions.
• Interpret and analyze patient data to determine status, management and treatment.
• Provide instructions and guidance regarding health care and health care promotion to patients and families.
• Other duties and responsibilities as assigned.
