Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider (APP) to deliver high-quality care within the scope of training and approved agreements, at a franchised AFC location. The APP will work collaboratively with other providers to ensure efficient patient flow and satisfaction, with responsibilities varying by franchise.

The role requires certification and licensure as a Physician Assistant or Nurse Practitioner, with board certification (ANCC, AANP, or NCCPA) and ER/UC experience

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate.

Responsibilities

• Perform complete, detailed health histories and reviews of patient records to develop medical assessments.
• Institute therapy or referrals to appropriate facilities and resources.
• Provide emergency measures and stabilization in critical situations.
• Interpret patient data to determine status and treatment.
• Educate patients and families on health care and promotion.
• Other duties as assigned.
