Job summary

American Family Care seeks an Advanced Practice Provider (APP) to care for patients within the scope of training and agreement, at a franchised AFC location. Work with other providers to ensure efficient patient flow and high satisfaction; duties may vary by franchise.

Responsibilities include detailed histories, diagnoses, therapy plans, emergency measures, and patient guidance. Licensed PA/NP with DEA and state certificates required; compensation ranges apply at franchise location.

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• Board certification and state licensure required for PA/NP.
• DEA number and state-controlled substance certificate as required.

Responsibilities

• Perform complete, detailed health histories and order appropriate studies.
• Formulate diagnoses and institute therapy or referrals as needed.
• Institute emergency measures and stabilization when required.
• Interpret patient data to determine status and treatment.
• Provide health care guidance to patients and families.
• Other duties as assigned.
