Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee and coordinate MDS assessments, CAAs, and care plans in a skilled nursing facility. The role requires adapting CMS RAI regulations to daily documentation and collaborating with an interdisciplinary team.

You will ensure timely submissions, educate staff on regulatory changes, and maintain high standards of patient care and compliance. CPS/CPR certification is required and experience in geriatric care is preferred.

Qualifications

• Associate's or Bachelor's degree in nursing from an accredited college or university.
• Active RN license in good standing throughout employment.
• Two years of nursing experience; geriatric preferred.
• CPR certification required by hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient's condition identified by the MDS Assessment to the DON.
• Educate direct care staff on CMS RAI Manual updates impacting documentation.
• Review Interdisciplinary Comprehensive Care Plan.
• Review CMS Reports and ensure timely billing readiness.
• Attend PPS Meeting and other required meetings.
• Perform staff nurse duties as needed.
• Maintain confidentiality and professionalism.
• Communicate effectively on an interdisciplinary team.
• Assist in patient evacuation and ensure safety.
