Job summary

Housing Options and Geriatric Association Resources Inc is seeking a dedicated Case Manager to provide services for residents in transitional housing. You will carry a caseload of 25 clients, coordinate housing and employment goals, and ensure regulatory compliance with funders.

You will perform assessments within 48 hours of arrival, manage CARES data entries, referrals for emergency services, and collaborate with housing coordinators to support independent living plans.

Qualifications

• Bachelor's degree in social work or a related field is required.
• One year of social service experience or approved equivalent.
• Familiarities with evidence-based practices and trauma-informed care preferred.
• Excellent organizational, written, and verbal communication skills are preferred.

Responsibilities

• Provide case management services for clients in transitional housing and manage a caseload of 25 clients.
• Conduct an initial assessment within 48 hours of arrival at the facility.
• Initiate referrals for necessary emergency services and coordinate CARES data entry.
• Maintain CARES data and documentation in the Uniform Client Chart and ensure it is current.
• Assist clients with income and public benefits and establish connections to medical/mental health services.
