Job summary

Responsibilities

Commits to evidence-based practices (EBP), MI, TIC, and boundary setting to help clients reach goals.
Conduct initial assessments within 48 hours of arrival at the facility.
Initiate referrals for emergency services and care.
Enter intake information in CARES data system and maintain client charts.
Provide case management including crisis intervention, counseling, and support services.
Develop Independent Living Plans, track progress, and ensure compliance.
Assist with income and public benefits applications and housing referrals.
Attend meetings, supervise, and perform mandated reporting as required.
