Job summary

Socket.dev is looking for a dedicated Case Manager to oversee case management for clients in transitional housing. You will manage a caseload of 25, guiding housing and employment goals while ensuring funder compliance.

Responsibilities include initial assessments within 48 hours, referrals for emergency services, CARES data entry, and comprehensive case management with crisis intervention and counseling. Strong organizational and communication skills are essential.

Qualifications

• Must have excellent organizational, written, and verbal communication skills.
• Familiarity with evidence-based practices (EBP), motivational interviewing, trauma-informed care is preferred.
• Experience with CARES data systems or similar case-management tools is a plus.

Responsibilities

• Commit to using EBPs, MI, TIC, CSTI, boundary setting to help clients achieve goals.
• Conduct initial assessments within 48 hours of arrival.
• Initiate referrals for emergency services and necessary supports.
• Maintain CARES data entry and supporting documents in client charts.
• Provide comprehensive case management including crisis intervention and counseling.
• Develop Independent Living Plans for each client and track progress.
• Assist clients with income and public benefits.
• Lead client orientations and review house rules, rights, and responsibilities.
• Maintain client data and ensure regulatory and contractual compliance.
• Attend meetings and provide mandated reporting as required.
