Collaborative Solutions for Communities in Washington, DC is seeking a Transitional Housing Case Manager to provide comprehensive case management to individuals and families in transitional housing, helping them achieve housing stability and self-sufficiency through assessment, planning, advocacy, and resource coordination.

You will conduct intake assessments, develop service plans, monitor progress, and collaborate with housing providers and partners.

Qualifikationen

• Bachelor's degree in social work, human services, psychology, counseling, sociology, or related field.
• Minimum two years of case management experience working with vulnerable populations.
• Strong organizational, communication, and documentation skills.
