Job summary

Housing Works seeks a skilled Case Manager for the Emergency & Transitional Housing Program (ETH) in New York. You will provide direct case management and housing placement assistance, including psychosocial assessments, plan development, and benefits enrollment, to help reentry individuals reintegrate into the community.

You will work with a team to coordinate services, maintain accurate records, and ensure adherence to program goals and quality standards.

Qualifications

• Bachelor’s Degree required in social services, psychology, social work or other related field.
• Experience providing case management or direct client service.
• Experience working with individuals involved in the criminal legal system, as well as individuals with substance use or mental health diagnoses.
• Strong written documentation and communication skills.
• Good problem-solving skills and ability to work autonomously.
• Microsoft Office Suite, particularly Excel, required

Responsibilities

• Manage a caseload of approximately 35-45 ETH program participants.
• Conduct psychosocial assessments for new clients.
• Develop housing and treatment plans with clients and review every 30 days.
• Assist clients with obtaining vital documents and benefits enrollment.
• Coordinate housing applications and scheduling viewings.
• Maintain client charts in EMR and participate in team meetings.
