Job summary

Community Outreach, Inc. seeks a Case Manager to provide case management for Transitional Housing and ASSETS programs, including intake, goal planning, referrals, client meetings, and documentation.

This full-time exempt position reports to the Director of Operations and requires confidentiality, collaboration with partners, and reliable records management. Evening and weekend hours may be required.

Qualifications

• Must have a Bachelor's Degree in a related field.
• Knowledge of issues concerning the homeless population and experience with low-income and homeless individuals/families is required or highly desirable.
• Excellent collaboration and relationship-building abilities with service providers.
• Strong organizational and communication skills.

Responsibilities

• Maintain professional attitude and behavior.
• Abide by confidentiality policies.
• Perform duties of Social Service Assistant.
• Conduct client intake interviews for Transitional Housing Program.
• Assist clients with identifying and pursuing objectives within the program.
• Conduct regular case management meetings with clients.
• Facilitate life skills or other group meetings.
• Maintain client charts per guidelines.
• Participate in multi-disciplinary staffing and present clients accordingly.
• Ensure clients understand and adhere to program rules.
• Assist with special events and presentations.
• Provide ongoing reports and recommendations to the Director of Operations.
• Gather and maintain program statistics.
• Process weekly, monthly and quarterly reports.
• Suggest program improvements to the Director of Operations.
• Contribute to ongoing program development.

Education

Bachelor's Degree in HDFS, Psychology, Social Work, or a related field
