Collaborative Solutions for Communities in Washington, DC is seeking a Transitional Housing Case Manager to provide comprehensive case management to individuals and families in transitional housing, helping them achieve housing stability and self-sufficiency through assessment, planning, advocacy, and resource coordination.
You will conduct intake assessments, develop service plans, monitor progress, and collaborate with housing providers and partners.
The Transitional Housing Case Manager is responsible for providing comprehensive case management services to individuals and families residing in transitional housing programs. The Case Manager assists participants in achieving housing stability, self-sufficiency, and personal goals through assessment, service planning, advocacy, crisis intervention, and coordination of community resources.
Bachelor’s degree in social work, Human Services, Psychology, Counseling, Sociology, or related field. Minimum two years of case management experience working with vulnerable populations. Strong organizational, communication, and documentation skills.
Experience in transitional housing, rapid re-housing, or supportive housing programs. HMIS experience. Knowledge of trauma-informed care, motivational interviewing, and harm reduction. Bilingual English/Spanish preferred.
Combination of office, field, and community-based work. Local travel may be required. Occasional evening and weekend hours may be necessary.
Salary: $65,000
