Qualifications

• Bachelor's degree in social work or related field.
• 1 year of social service experience or equivalent.
• Familiarity with EBPs such as strength-based case management and MI.
• Knowledge of entitlement systems and CARES data entry preferred.

Responsibilities

• Manage a caseload of 25 clients in transitional housing.
• Conduct initial assessment within 48 hours of arrival.
• Develop Independent Living Plans and track progress.
• Provide crisis intervention, counseling, and referrals.
• Complete CARES data entry and maintain client charts.
• Attend meetings and coordinate with housing staff.
