Housing Options and Geriatric Association Resources Inc. in The Bronx, NY is seeking a Case Manager to support residents in a supervised housing program.

You will conduct initial and ongoing assessments, develop individualized service plans within 28 days of admission, monitor progress, provide skill training, and advocate for residents to access income assistance and community services. A Bachelor degree in a human service field is required.

• Participate in intake for admission to the supported housing program.
• Conduct monthly assessments to determine resident functioning, needs and strengths.
• Develop initial individualized service plans within 28 days after admission with residents.
• Record progress of residents within 25 hours of service delivery and after each contact.
• Make referrals to local supportive service providers and advocate for residents to ensure timely benefits.
• Attend and participate in case conferences and coordinate with other providers.
