Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% in the Metro East, Illinois area with mileage reimbursement.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members and determine needs, eligibility and care plan approaches.
• Identify high-risk factors and refer to clinical case management as needed.
• Coordinate and monitor care plan activities and progress.
• Collaborate with medical directors and teams to improve quality of care.
• Engage in team meetings and mentor peers.
