Job summary

Qualifications

• Willing to travel 50-75% in Mason, Cass, Scott and Brown counties, Illinois.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Coordinate and monitor care plan activities and progress.
• Evaluate member needs/eligibility and inform care planning.
• Collaborate with clinicians and health programs to overcome barriers.
• Engage in team meetings and provide peer mentoring.
