Job summary

Qualifications

• Experience with electronic documentation and Microsoft Office applications.

Responsibilities

• Evaluate members’ needs and eligibility and recommend case resolution.
• Coordinate and implement care plan activities and monitor progress.
• Escalate quality of care issues through established channels and collaborate with care teams.
• Provide coaching and support to empower members in health decisions.
• Engage in team meetings and mentor other staff.
