Job summary

CVS Health is seeking a Case Management Coordinator for a full-time telework role with 50-75% travel in the Metro East/Southern Illinois region. You will support members by scheduling appointments, guiding benefits and education, and using care management tools to inform care decisions.

The position offers standard hours Monday–Friday 8:00am–5:00pm CST and includes a New Hire Bonus of $3,000. A Bachelor’s or Master’s in Behavioral Health or related field is required, with 2+ years in the field

Qualifications

• 2+ years experience in Behavioral Health or equivalent field.
• Willing to travel 50-75% of the time in the Metro East Illinois area.
• 2+ years of electronic documentation experience and MS Office usage.

Responsibilities

• Evaluate members' needs and eligibility and plan care resolution.
• Coordinate and monitor care plans and progress.
• Collaborate with clinical teams to overcome barriers to goals.
