Qualifications

• 2+ years experience in Behavioral Health or related field.
• Willing to travel 50-75% around Southern Chicago, IL.
• 2+ years experience with electronic documentation and Microsoft Office.

Responsibilities

• Evaluate members and assess needs and benefits for care planning.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Contribute to quality of care and case conferences for multidisciplinary review.
• Escalate quality of care issues as needed.
• Coach and empower members to participate in healthcare decisions.
• Participate in team meetings and peer mentoring.
