Qualifications

• 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus.
• Must be willing and able to travel 50-75% of the time in Will, Kankakee and Grundy Counties and surrounding areas, Illinois.
• Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluation of Members: through care management tools and data review, assess needs and eligibility and recommend an approach.
• Identifies high risk factors and service needs affecting outcomes and makes referrals.
• Coordinates and implements care plan activities and monitors progress.
• Collaborates with Medical Directors and health programs to improve care quality and outcomes.
• Identifies and escalates quality of care issues.
• Negotiates options and services to meet benefits and healthcare needs.
• Provides coaching and support to empower members in health decisions.
• Engages in team meetings and peer mentoring.
• Supports member participation with providers in decision-making.
• Documents care and quality management activities per guidelines.
