Responsibilities

• Assess and evaluate member needs and determine care resolution and benefits.
• Identify high-risk factors and coordinate appropriate referrals to care management.
• Coordinate and monitor care plan progress and outcomes.
• Escalate quality of care issues through proper channels.
• Provide coaching and support to empower members in healthcare decisions.
• Engage with team meetings and provide peer mentoring.
