Job summary

CVS Health is seeking a Case Management Coordinator to support members by scheduling appointments, guiding benefit access, and education through care management tools. This is a full-time telework role with 50-75% travel around Chicago, IL, and regular weekday hours.

You will evaluate member needs, coordinate care plans, and collaborate with clinicians to improve outcomes, while engaging with teams to promote self-management and quality care in a dynamic healthcare environment.

Qualifications

• Bachelor's degree in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy or Counseling).
• 2+ years of experience in Behavioral Health, Social Services or related field.
• Experience with electronic documentation and MS Office suite.

Responsibilities

• Evaluate member needs/eligibility and plan care resolution with appropriate referrals.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with case managers, Medical Directors and health programs to resolve barriers.
• Provide coaching and support to empower members in healthcare decisions.
