Service Coordination, Inc. is seeking dedicated Supports Planners to serve in Gaithersburg, MD, delivering person-centered case management and supports planning for individuals with disabilities and complex needs.

The role emphasizes advocacy, coordination of services, and ongoing POS development within a community-focused team. The position requires a relevant degree in psychology, social work, nursing, counseling, sociology, or human services administration, plus a valid driver's license.

Qualifications

• Bachelor's or master's degree in one of the specified human service fields.
• Experience working with people with disabilities preferred.

Responsibilities

• Advocates on behalf of people served and educates them to become more self-determined.
• Plans for and updates Person Centered Plans of Service (POS).
• Coordinates resources and services to achieve POS outcomes for people served.
• Monitors POS and conducts follow-up activities.
• Adheres to SCI policies and HIPAA guidelines.
