Qualifications
- Bachelor's or master's degree in one of the following concentrations: Psychology, Sociology, Social Work, Nursing, Counseling, Human Services Administration.
- Valid Driver's License and an insured vehicle.
Responsibilities
- Provide Case Management services and support to people who have a disability and/or medical need. Establish POS and plans with service providers; monitor progress and outcomes.
- Coordinate and advocate for supports, resources, and services; maintain HIPAA compliance; participate in team training and meetings.
