Job summary

Qualifications

• Minimum of 1 year experience in case management with people with complex medical and/or behavioral health needs.
• Must be a licensed driver with reliable transportation, valid insurance, and a good driving record.
• Bachelor’s or Master’s degree in human services (psychology, social work, sociology, nursing, counseling, or related field) or equivalent work experience.
• U.S. citizen or legally authorized to work in the U.S.
