Job summary

Qualifications
- Bachelor's or master's degree in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
- Valid Driver's License and an insured vehicle.
Responsibilities
- Advocates on behalf of the people served and educates them to become more self-determined.
- Obtains information and documentation related to eligibility for Supports Planning services and POS development.
- Plans for, and updates Person Centered Plans of Service (POS) in collaboration with people served.
- Coordinates resources and services to achieve POS outcomes and transitions to community supports.
