Qualifications

• Bachelor's or master's degree in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
• Valid Driver's License and an insured vehicle.

Responsibilities

• Advocate for people served, provide resources, and educate for self-determination.
• Collect information and documentation for eligibility and POS.
• Plan, facilitate, and update Person Centered Plans of Service (POS).
• Research and coordinate resources to achieve POS outcomes.
• Monitor POS and conduct follow-up visits.
• Ensure HIPAA compliance and follow policies.

Education
