Qualifications
- Education at bachelor's or master's level in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
- Experience in case management and working with people with disabilities is preferred.
- Valid driver’s license and insured vehicle are required.
Responsibilities
- Advocate for people served and educate them to become more self-determined.
- Collect information and determine eligibility for Supports Planning services.
- Plan, develop, and update Person Centered Plans of Service.
- Coordinate resources and services to meet POS outcomes.
- Monitor POS and conduct follow-up activities.
- Comply with HIPAA and organizational policies.
