Service Coordination, Inc. is seeking Supports Planners to work with communities and advocate for underserved individuals. The role involves coordinating supports planning, case management, and providing compassionate service while meeting program requirements.

Requirements include a bachelor’s or master’s degree in relevant fields and a valid driver's license with insured vehicle. Travel is reimbursed and a quarterly bonus is possible.

Qualifications

• Bachelor's or master's degree in psychology, sociology, social work, nursing, counseling or human services administration.
• Valid Driver's License and an insured vehicle.
• Experience with disabilities and person-centered planning preferred.

Responsibilities

• Support Planners in advocating for underserved individuals within the community.
• Coordinate supports planning and case management activities.
• Participate in training and maintain appropriate program documentation.
