Qualifications

Education requirement: Bachelor's or Master’s degree in Human Services related field.
Experience requirements: relevant case management experience preferred.

Responsibilities

Provide case management and support to people with disabilities or medical needs.
Establish POS and person-centered plans; monitor progress and outcomes.
Coordinate services, referrals, and resources; advocate for clients.
Maintain HIPAA compliance and accurate documentation.
