Total Care Services, Inc. recruits for a Supports Planner to empower clients with disabilities to live independently at home and in the community.

The role coordinates services, identifies waiver eligibility, and supports clients in planning and accessing needed supports. Ideal candidates will have 1+ year in case management for complex medical/behavioral needs, and must be licensed drivers with reliable transportation and solid communication skills.

Qualifications

• Minimum 1 year in case management for people with complex medical/behavioral health needs.
• Must be a licensed driver with reliable transportation, valid insurance, and a good driving record.
• Bachelor’s or Master’s in human services: psychology, social work, sociology, nursing, counseling, or related field.
• U.S. citizen or legally authorized to work in the U.S.
• Pass a criminal background check.
• Flexible to work from home and/or community, and respond to crisis nights/weekends.
• Effective written and oral communication skills.
• Excellent organization and time management skills.
• Proficient in using Microsoft Office; ability to learn new technologies.
