Job summary

PATH is seeking a Case Manager for the Metro Villas site in Los Angeles, CA to assist clients in breaking the cycle of homelessness. The role focuses on housing goals, health needs, and access to supportive services using a coordinated entry system (CES).

You will provide individualized supportive services, develop ISP plans, and connect clients to healthcare, social services, benefits, and housing options within PATH’s SHS program. On-site work is required and driving is expected.

Qualifications

• Two years of experience in healthcare, case management, homeless services or related field.
• CPR/First Aid training or ability to obtain
• Background screening, Tuberculosis test and drug test required
• Employment eligibility to work in the United States

Responsibilities

• Conducts intake and develops an ISP addressing housing goals and income and other personal goals identified by the participant.
• Provides referrals, linkages, information, and support to resources to achieve ISP goals
• Completes program specific assessments (e.g. VI-SPDAT)
• Evaluates strengths and challenges, conducts a 90-day review with the participant
• Maintains documentation standards and keeps a complete participant file with required documents
• Maintains confidentiality of participant files per HIPAA and guidelines
• Develops therapeutic relationships and provides crisis intervention and trauma-informed care
• Uses motivational interviewing to support clients in housing and health goals
• Keeps knowledge of community resources to provide wrap-around services
