Job summary

PATH (People Assisting The Homeless) is seeking a Case Manager in Los Angeles, CA. The role focuses on helping clients break the cycle of homelessness through individualized service planning, housing goals, and coordinated care across health, social, and housing systems.

You will engage in intake, ISP development, referrals, and ongoing support to improve housing stability and income, while maintaining client confidentiality and collaboration with partners.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and linkages to resources to support ISP goals.
• Complete program assessments (e.g., VI-SPDAT) and reviews.
• Evaluate strengths/challenges; conduct 90-day review with participants.
• Maintain accurate case files and confidentiality per HIPAA.
• Develop relationships with clients and systems to sustain housing.
