Job summary

PATH is seeking a Case Manager - SSVF Housing Retention in Los Angeles, CA to support clients through housing transitions, access essential services, and coordinate individualized plans. You will conduct intake, assess needs, link resources, and document progress to prevent homelessness.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income
• Provide referrals and support to resources to achieve ISP goals
• Complete program specific assessments (e.g. VI-SPDAT)
• Evaluate progress with 90-day reviews and adjust plans accordingly
• Maintain complete and accurate participant files and documentation
