Job summary

The People Concern seeks a Case Manager for our 1603-Housing Retention program, based onsite at Vermont Corridors, LA. You will support clients’ housing stability, self-sufficiency, and community integration using evidence-based practices in a trauma-informed, housing-first framework.

You will manage a high-acuity caseload, coordinate with DMH & DHS, assist with benefits, and connect clients to health and social services to promote sustainable housing outcomes.

Qualifications

• High School Diploma or GED or Equivalent.
• 1 year of experience with homeless individuals and a social work/mental health bachelor’s degree, or 2 years of direct mental health/intensive case management experience.
• CA Driver’s license, reliable car, auto insurance, and a clean driving record.
• Knowledge of issues faced by culturally diverse populations.
• Proficiency in Word, Excel and Outlook.

Responsibilities

• Maintain a caseload of approximately 20 high acuity Clients for intensive case management services (up to 40 if low acuity).
• Develop relationships with Clients with a focus on independence and health maintenance.
• Collaborate with LA County DMH and DHS programs to coordinate treatment.
• Engage Clients with harm-reduction and trauma-informed care approaches.
• Assist in screening, intake, and enrollment of Clients and orientation to policies.
