The Case Manager works within our Supportive Housing Services to provide Intensive Case Management Services in a Permanent Supportive Housing setting to individuals who have experienced homelessness and have chronic health, mental health, or co‑occurring conditions. Services are centralized and provided on-site to support housing stabilization, quality of life, and community integration.
