PATH is seeking a dedicated Case Manager in Los Angeles to help clients navigate housing systems and health services. You will perform intake, needs assessments, and individual service planning, coordinating with healthcare and social supports to address housing goals and income.

The role emphasizes trauma-informed care, crisis intervention, and wrap-around services, with a focus on confidentiality and ethical practice. A full-time, non-exempt schedule is typical.

Responsibilities

• Conduct intake and needs assessment to develop ISP addressing housing goals and income.
• Provide referrals and support to resources to achieve ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Evaluate progress and conduct 90-day reviews with participants.
• Maintain accurate documentation and confidential files per HIPAA policies.
• Build therapeutic relationships and coordinate with partners to support housing goals.
• Provide crisis intervention and trauma-informed care as needed.
• Maintain knowledge of community resources and wrap-around services.
