Job summary

PATH is seeking an Outreach Case Manager in Los Angeles, CA to help clients break the cycle of homelessness through individualized service plans and comprehensive wrap-around services. You will coordinate housing, health, and social supports, ensuring access to benefits and community resources within a trauma-informed framework.

Responsibilities

• Conducts intake and develops individualized service plans addressing housing goals.
• Supports participants to increase income and access healthcare and social services.
• Provides referrals and connects clients to resources to meet ISP goals.
• Completes program assessments (e.g., VI-SPDAT) and 90-day reviews.
• Maintains client files and documentation per HIPAA and PATH policies.
• Delivers crisis intervention and trauma-informed care.
• Builds therapeutic relationships to support housing stability.
