Job summary

SPECIAL SERVICE FOR GROUPS, INC is seeking an On-Site Case Manager to support Pathway Home participants, screening and managing cases on-site and coordinating housing transitions under supervision.

You will work with DHS, LAHSA, and partner agencies, enter data in CLARITY/HMIS, and prepare reports. An associate degree in social services or related field is required, with strong communication and Word/Excel skills.

Qualifications

• Associate degree in social services or related field required; 1–2 years homeless program experience preferred.
• Ability to communicate effectively, both written and oral.
• Working knowledge of Microsoft Word and Excel; experience with databases.
• Experience with HMIS is preferred.

Responsibilities

• Screen and provide on-site case management for Interim Housing participants.
• Coordinate services and referrals with the supportive services team.
• Enter data into CLARITY/HMIS within 24 hours.
• Prepare program reports and participate in team meetings.
• Conduct life skills training and group sessions.
