Job summary

PATH (People Assisting the Homeless) is hiring a Case Manager for the Metro Villas site in Los Angeles, CA. This on-site role supports clients across housing phases, coordinating access to healthcare, social services, and benefits to achieve housing stability.

Responsibilities include intake, ISP development, referrals, crisis intervention, and ensuring confidentiality under HIPAA. Driving is required; full-time with a daytime schedule.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and linkages to resources to achieve ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Evaluate progress and conduct 90-day reviews with participants.
• Maintain documentation per PATH policies and HIPAA guidelines.
• Develop therapeutic relationships and coordinate wrap-around services.
• Provide crisis intervention and risk assessment, supporting safety.
• Utilize motivational interviewing and trauma-informed care.
• Keep knowledge of community resources current for client needs.
