Zusammenfassung

PATH (People Assisting the Homeless) seeks a Case Manager in Los Angeles, CA to support clients across housing stages from interim housing to permanent housing. You will conduct intake, develop individualized plans, provide referrals, and coordinate services while maintaining confidential records.

Non-exempt, full-time, Monday–Friday, with driving as needed. The role emphasizes trauma-informed care, motivational interviewing, and collaboration with service partners to promote housing stability

Qualifikationen

• Analytical skills with attention to detail to interpret data and inform decisions.

Aufgaben

• Conducts intake and develops an ISP addressing housing goals, income, and barriers.
• Provides referrals and linkages to resources to achieve ISP goals.
• Completes program assessments (e.g. VI-SPDAT).
• Maintains documentation standards and participant files per PATH policies.
• Maintains confidentiality of participant files per HIPAA and guidelines.
• Develops therapeutic relationships with clients and partners to support housing goals.
• Provides crisis intervention and risk assessment services to increase safety.
• Uses motivational interviewing and trauma-informed care in service delivery.
