Job summary

PATH seeks a compassionate Case Manager for the Villas at Gower in Los Angeles, CA. You will assist clients to break homelessness cycles, coordinating health, housing, and social services, and helping them develop personalized plans.

Responsibilities include intake, ISP development, referrals, and crisis intervention, with a focus on housing stability and client self-advocacy. This is a full-time, on-site position requiring driving and community engagement.

Qualifications

• Two years of experience in healthcare, case management, homeless services or related field.
• CPR/First Aid training required.
• Employment eligibility verification required.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and support to resources for ISP goals.
• Complete program assessments (e.g. VI-SPDAT).
• Evaluate strengths and challenges; conduct 90-day review.
