Job summary

Care Lync is seeking a dedicated Support Coordinator to deliver person-centered assessments and develop ISP plans for adults with disabilities and aging individuals. You will collaborate with medical staff, family, and community resources to secure appropriate services and promote independence.

Travel to participants' homes and communities is required. The role emphasizes advocacy, confidentiality, and ongoing professional development within a supportive team culture.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
• Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program and ethical standards.
• Participate in ongoing training and professional development to strengthen skills and impact.
