Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and the aging population access housing, medical care, and community services. You will develop individualized plans, coordinate with providers, and advocate for participants to ensure safety and independence.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify each participant's needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
• Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
