Qualifications

• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify needs, goals, and preferences.
• Develop and update ISP plans to ensure participants receive the right services.
• Coordinate with medical professionals, family, and resources to deliver care.
