Job summary

Care Lync is seeking a dedicated Support Coordinator to empower adults with disabilities and the aging population. You will coordinate services, develop and monitor ISP plans, and advocate for client choices to secure appropriate care options.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Ability to travel up to 50% to meet participants in homes and communities.
• Valid driver's license and auto insurance are required.

Responsibilities

• Conduct person-centered assessments to identify needs, goals, and preferences.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy for providers and care options.
• Coordinate with medical professionals, families, and community resources.
• Monitor services to ensure safety, satisfaction, and progress toward independence.
• Support adults with disabilities and aging populations in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Participate in ongoing training and professional development.
