Job summary

Care Lync seeks a Support Coordinator to empower adults with disabilities and seniors by coordinating services, developing ISP plans, and advocating for participant choices. Travel to participants’ homes may be required to ensure access to housing, medical care, and community resources.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Willingness to travel up to 50% to meet participants in homes.

Responsibilities

• Conduct person-centered assessments to identify participant needs, preferences, and life goals.
• Develop, implement, and update Individual Support Plans (ISP) to ensure appropriate services.
• Provide participants with choice and advocacy in care options.
• Coordinate with medical professionals, behavioral specialists, family, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Support adults with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Participate in ongoing training and professional development.
