Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and the aging population access housing, medical care, and community resources. You will conduct person-centered assessments, develop and update ISP, and advocate for participants to ensure informed care choices.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Willingness to travel up to 50% to meet with participants in their homes and communities.
• Valid driver's license and auto insurance.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver high-quality care.
• Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
• Support both individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with all program and ethical standards.
• Participate in ongoing training and professional development to strengthen skills and impact.
