Job summary

Care Lync is seeking a dedicated Support Coordinator to provide case management and develop individualized care plans for adults with intellectual, developmental, and physical disabilities. You will broker and monitor services, ensuring outcomes align with ISP goals and are cost-effective.

The role requires strong coordination across providers, adherence to confidentiality, and the ability to travel to participants up to half the time. A BS/BA degree and relevant certifications are required.

Qualifications

• BS/BA degree required.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and state clearances.
• Valid driver's license with vehicle insurance and registration.
• Complete required trainings and certifications.

Responsibilities

• Complete assessments to identify participant needs for service delivery.
• Develop a Person-Centered Service Plan and update annually or as needed.
• Provide information to help participants choose providers.
• Ensure ISP aligns with Behavioral and Crisis Plans.
• Report incidents of abuse, neglect or exploitation promptly.
• Coordinate with other care entities to ensure seamless services.
• Research and develop alternative solutions for participant needs.
• Maintain confidentiality of sensitive information.
• Attend trainings and recertifications as required.
• Communicate with supervisors to consider participant needs and limitations.
• Travel to participants as needed (approximately 50%).
