Job summary

Care Lync in Maine is seeking a dedicated Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, and community resources, while promoting independence and dignity.

You'll conduct person-centered assessments, develop ISP plans, advocate for participants, and collaborate with medical professionals and families. Travel up to 50% may be required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• 3+ years of experience providing case management or support coordination to individuals with disabilities and/or aging adults.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling up to 50% to meet participants in homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and life goals.
• Develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
• Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging adults in accessing community resources.
• Maintain documentation, confidentiality, and compliance with all program and ethical standards.
• Participate in ongoing training and professional development to strengthen your skills and impact.
